HighPointe on Michigan (HighPointe) was the bronze winner of the 2017 McKnight’s Technology Awards, quality category, for their use of a software system developed by Patient Pattern, Inc. to improve patient quality outcomes. Patient Pattern is a program developed by Steven Buslovich, MD, medical director of HighPointe. The McKnight’s Technology Awards recognizes providers who have harnessed technology to make life better for their residents.
By using this innovative software to address care management of a clinically complex population, HighPointe was able to reduce quality outcome metrics by 50 percent. Over the last year, HighPointe has decreased the number of Stage 3 or 4 pressure ulcers in the facility from 64 to 16, decreased physical restraints from 40 to 0, decreased urinary tract infections by 80 percent, and decreased re-hospitalizations from the national average of 20 percent to 13 percent. Based upon these changes, HighPointe has improved its CMS Nursing Home Compare Quality Ratings from 2-stars to 4-stars in the past year. Also, given the facility’s Quality Measure trends for 2017, HighPointe will become a 5-star rated facility for quality in early 2018.
The software program uses data collected from hospital records and patient or family interviews to compare each patient to thousands of previous cases and assess the patient’s risk for recovery. These risks are matched with evidence-based recommendations which provide HighPointe clinicians with a proactive clinical care approach for managing these risks.
HighPointe clinicians and staff significantly benefit from this evidence-based approach and decision support provided by this software and are able to frame realistic risk and benefit conversations with the patients and families. The use of this care software is truly an asset to patient care for both the clinicians as well as the patient. The overwhelming success that HighPointe has seen in just the first year of use with this software proves that the use of technology can have a drastic positive impact on the care of patients, patient satisfaction and facility bottom-line.
